July 20-August 3rd : $400
August 3rd-August 12th: $500

***Registration does not include equipment or jersey. Please select the uniform option you need during registration. Jerseys from 2023 season will be used for 2024 season 
***Buy your equipment early to ensure availability. 

• I am not aware of any injury, illness or other health related issues that would restrict or limit my child’s ability to play or train for competitive sports.

• I agree to assume all risks and expenses due to any injury that may occur because of my child’s participation in practice, training and/or athletic sessions with the PW-Tri Town Golden Eagles

• I agree to hold PW-Tri Town Golden Eagles staff and trainers harmless in the event of any injury to my child while participating under the supervision of the above

• PHOTO DISCLOSURE: This document confirms the agreement between you and PW-Tri Town Golden Eagles regarding you and/or your athlete’s participation in approved PW-Tri Town Golen Eagles. Performance activities in which you and/or your athlete may be photographed and/or videotaped throughout their above agreed upon event(s) with the potential to be shared within PW-Tri Town Golden Eagles social media platforms.
